Lens Repositioning or Exchange
When a lens is dislocated, decentered, or not the right optical match, vision can remain distorted or blurry. Through precise microsurgical techniques, Dr. Tokuhara repositions or replaces the intraocular lens (IOL) using advanced optics and intraoperative aberrometry. This allows him to create accurate alignment, improved focus, and lasting visual stability.
Iris Repair and Reconstruction
Trauma or prior surgery can leave the iris uneven or damaged, causing light sensitivity and visual imbalance. Secondary Lens Implant
Some patients may need a new lens implant after previous eye surgery, trauma, or retinal surgery where the natural lens was removed. Depending on the condition of the eye and the remaining support structures, different types of lens implants may be used safely and effectively.

For Patients:
R.E.F.O.C.U.S.™ is a concierge-level surgical program designed for patients who need corrective cataract surgery after a complication, poor outcome, or complex diagnosis. It includes advanced diagnostics, record review, and customized surgical repair using techniques such as lens exchange, scleral fixation, and iris repair.
If you have blurry or distorted vision after cataract surgery—or if you’ve been told your case is “too complex” or “not fixable”—you may benefit from RE.F.O.C.U.S.™. We also see patients with dislocated IOLs, visual distortion, iris damage, or other post-surgical concerns.
No referral is required. While many patients are referred by other doctors, you can contact us directly to schedule a consultation.
Depending on your unique needs, Dr. Tokuhara may use:
- Yamane Scleral Fixation (sutureless IOL fixation)
- Canabrava Double-Flanged Technique
- Iris repair or pupilloplasty
- Anterior vitrectomy or IOL repositioning

RE.F.O.C.U.S.™ is a structured, concierge-level program at Desert Vision Center for patients who require cataract revision surgery, lens repositioning, or post-surgical complication repair. It includes comprehensive diagnostics, a forensic surgical review (when prior records are available), and a tailored surgical plan performed by Dr. Keith Tokuhara.
We welcome referrals for:
- Dislocated or subluxated IOLs
- Incorrect lens power or selection
- Post-surgical visual disturbances or glare
- Iris damage, pupil abnormalities, or traumatic cataracts
- After-LASIK or post-RK cataracts requiring special planning
- Advanced or high-risk cataracts with zonular instability, pseudoexfoliation, Fuchs’, etc.
We also provide second opinions for patients previously told their case is “unfixable” or “too complex.”
